An effective consultation is a vital part of engaging with individuals, organizations, and communities in healthcare system navigation process with the aim of improving healthcare standards for all stakeholders.

For an effective consultation, the model consists of the following stages:

 

  • Inform

            - The focus is to maintain and improve the patient’s health. By keeping               the patient informed and educated, the patient can An informed and                       educated patient can participate in his or her own care, improve                             outcomes, and help identify discrepancies in care. In providing the                         patient an opporutnity to succeed.

  • Involve 

            - Patient involvement goes beyond patient client discussion.                                  Involvement means having the patient voice heard at every level of the                 service, even when that voice is a whisper. Evidence shows that patient                 safety improves when patients are more involved in their care and have                 more  control. Patient involvement is crucial to the delivery of                                 appropriate, meaningful and safe healthcare and is essential at every                     stage of the care cycle: at the front line, at the interface between                           patient and clinician; at the organizational level; at the community level and

at the national level.

  • Collaborate 

            - Patient collaborative care involves engaging any health provider whose           expertise can help improve the patient/resident/client’s health. It also                   includes the patient/resident/client and their family as members of the                 health-care team.
